Need Web Based Software application

I am looking for software that can be run from a web page that will be an online test.

This is what it should do.

1. Fields for member to enter members information such as name reference number telephone number and so on

2. Have multiple selection questions and answers
Example
Q What day is it (select correct answer from list)
Monday
Tuesday
Wednesday

So basically you have a number of questions and answers like this and the user ticks what they thinks is the correct one for each question. At the end of the quiz it needs to file a mark according to the number of correct answers given

Any help with this will be greatly appreciated as I have no idea where to start looking for something like this
