In this case, I think it was the show itself. The video quality for the first season of Sanctuary was terrible when streamed, but I started streaming the first season of Lost immediately after that and it was truly fantastic.
My local post office seems to have mail goof ups all the time, but in this case I think something happened at the originating post office causing the DVD to be returned to the sender. Netflix was quick on their feet and have already shipped it out again.The mail problems are due to the holidaysoz

another quick comment about Netflix
One thing that I really like about Netflix is the way they communicate to me everything that's going on with my account.
They send an email each time a disk is mailed, and another comes when they've received a disk. Any changes to my account are also documented and emailed to me. Of course, all this is outlined on your personal account webpage, but it's nice to have the emails as backup evidence of these things should they ever be needed.
Their website is really easy to understand and use but it does seem to run a little slowly on occasion.
I've definitely decided to keep the account after the free trial has ended in a few more days!

It appears that Netflix subscribers are about to have a lot more choice when it comes to streamed video:

My wife and I have been pleasantly surprised at how honest they are about lapses in service as well. Most of the time we never notice when one of their regional warehouses has a "down" day, but when it does they send us an email with either a free movie from our queue (that doesn't count against our total) or a pro-rated refund credit for that day's service.

for Netflix streamers
For those of you into Netflix streaming, I've run across a couple of websites that might make it easier for you to find what is available through streaming right now, or soon to be streamed:

Edit: Unfortunately, I've been a little bit unhappy with their netstreamed video services, so far. My connection is 10 Mb/s which seems that it should be fast enough, but at least half of the videos that I try to stream from them are either overly blurry and of poor quality, or the transmission seems to break now and then. With other videos played during the same session, they look almost as good as blu-ray does. This observation is the result of at least 25 movies that I've streamed from Netflix thus far. Hopefully, things will improve soon and stay that way.
On the other hand, their Blu-ray service has been great so far. Others have reported receiving Blu-ray disks that are scratched and won't play, but all of them that I've received thus far look brand new, or they have very few scratches and play just fine on my player.
