I had this problem for the longest time and the only thing that fixed it was I finally did a reinstall for different reasons and then it worked. I have absolutely no idea why the sound wasn't working or why it began to work after this but maybe re-emerging some stuff will help.
try re-emerging firefox
netscape-flash

i found out that if i play music while firefox is up and try to watch/listen to something with flash it has no sound, so i just restart firefox and turn off xmms and it worked
