If that's a Connexant HSF modem then you need linuxant drivers from www.linuxant.com. The exact link to installation insturctions is http://www.linuxant.com/drivers/hsf/install.php
You can download the driver from there, but it'll only work on low 14k speed. To have full speed Linuxant asks you to pay a one time fee. But then again, the fee is almost as high as the new, real modem to buy that'll work on GNU/Linux.
What you have is a softmodem, which tend to have issues with GNU/Linux.
There is a workaround to make Connexant HSF work fully, but its legality is doubtful so.. I don't know if I should share it with you.. maybe you could find it yourself on google if you must. It involves downloading the driver from Linuxant, patching it with hsfpatch and entering fake email and full code.....
