You have posted two outputs of fdisk -l command and both show different partition structure of 160GB Hard disk. First output shows 2 NTFS partitions ( +1 External ) and second output shows 1 NTFS and 2 Linux partitions ( +1 External ).

Which output is current partition structure of your Hard disk?

Boot up from Linux Mint LiveCD and open Disk Utility package.
Delete /dev/sda5 partition and create 2 new partitions.

    /dev/sda5 - 2GB, SWAP
/dev/sda6 - ext4, assign as much space as you like. 15GB is more than enough for Desktop User.

You can create a few more partitions for Data Sharing. Linux supports NTFS read/write access out of box.

Start installation and select Manual Partitioning in Partition section. Select ext4 partition and assign it / mount point. Installer will detect SWAP and Windows OS partitions. Installer will setup dual boot correctly.
