I downloaded 2 packages: cxacru_bundle-2004-05-11 and usbatm-20050216 and then installed driver, firmware for modem. It work
When I ran ~/cxacru/scripts/cxstart.sh (from cxacru_bundle-2004-05-11) ...

nas0 error : no such device (adsl usb modem)

When I ran ~/cxacru/scripts/cxstart.sh (from cxacru_bundle-2004-05-11) and see this :

I found ADSL modem with VendorID = 0572 & ProductID = cb00

Error in ioctl call, status = -1
I know the error in "ioctl call" is: nas0 can't create, no such device (because I ran br2684ctl and see this messenge)
So how can I create a nas0 interface in manual or I miss something
I use ADSL USB Modem in FC2
