You will probably need to give more information before anyone can help you.
What version of Open Office are you running?
What is Form Wizard doing, or not doing?
Are you getting error messages, or is it crashing, or what?oz

Ooo 2.0 and the wizard form opens when clicked on but doesn't have the wizard which to select options. No error messages.Thanks

If it was working before but isn't now, the only thing I can think of that might fix it is to shutdown OpenOffice, then rename your hidden openoffice folder in your /home/username directory, then restart OpenOffice to let the folder rebuild itself. Keep in mind that you will loose your OpenOffice settings and will have to reset them.
If this doesn't work, you can delete the new OpenOffice folder and re-implement the old one that contained your previous settings.
