I got this error from /var/log/messages from one of our server :
May 20 10:17:29 minnie cmaeventd[3808]: Logical drive 1 of Array Controller in slot 1: I/O request fatal error.

and when i checked with command # dmesg, this line is keep looping :
cciss: cmd c7580000 has CHECK CONDITION byte 2 = 0x3

Does someone know what happen? is it a broken disk?
