I use Sendmail. It took a small amount of effort to configure, but was relatively straightforward. I run multiple domains and multiple users through mine. See www.sendmail.org, configuring: http://www.sendmail.org/m4/readme.html, relaying (or control of): http://www.sendmail.org/tips/relaying.html, and anti-spam: http://www.sendmail.org/antispam.html
