How remove app from running on startup?

Generally KDE will restart any app that is running when you end the session. IIR there is a option in the configuration that allows you to change this.

    Personal settings-KDE components- Session manager

    check start with empty session

Where do I find "Personal Settings"?

And I wasn't using KMail. I've never used it, so that's why it's odd it was starting it up.

Must have been open at some point when you shut down

As to the Personal setting, look in the menu. It may be just setting it depends on the os version. Basically you are looking for the control center. This is where you configure most of KDE.
